Applicants may use the Daemen <a href=\"http:\/\/www.daemen.edu\/sites\/default\/files\/Daemen%20AT%20student%20Observation%20Record%20Form.xls\">Athletic Training Student Observation Record Form<\/a> available on the Athletic Training website or submit a letter signed by a BOC Certified Athletic Trainer with dates, number of hours, and setting.<\/li>\n<li>Applicants must submit two character references (one must be completed by a BOC Certified Athletic Trainer).
